Betamethasone, a potent synthetic corticosteroid, exerts its anti-inflammatory and immunosuppressive effects by binding to cytoplasmic glucocorticoid receptors. Once bound, the corticosteroid-receptor complex migrates to the nucleus, where it regulates gene expression by inducing the synthesis of lipocortin, which inhibits phospholipase A2 and reduces the formation of pro-inflammatory mediators such as prostaglandins and leukotrienes. Additionally, betamethasone suppresses the production of various inflammatory cytokines, including interleukins and tumor necrosis factor-alpha (TNF-α). Gentamicin, an aminoglycoside antibiotic, works by binding to the 30S ribosomal subunit of susceptible bacteria, leading to mistranslation of bacterial proteins and disruption of bacterial cell wall synthesis. This binding interferes with protein synthesis, causing errors in the reading of mRNA, which ultimately leads to bacterial cell death. The combination of betamethasone and gentamicin in the ointment provides both anti-inflammatory and antibacterial effects, making it effective in treating inflammatory skin conditions with secondary bacterial infections.
